Hi,
I have reviewed both images. The discoloration is more suggestive of a longstanding reaction to something the body doesn't tolerate. If he applies any special lotion to this area, even for treatment purposes, I will suggest that he stops. They are clearly not caused by trauma.
I will like to see what this discoloration looks like at least 1week after nothing special is being done to it. This would be the best way to get this assessed. At this time, it presents no threat to life and we rather be gentle and logical than go out and be aggressive from the start.
For how long has this been going on? Is he having any other health concern?
