Suggest Treatment For Dark Spots And Pigmentation Due To Acne

Question: Hello Doctor,
I am 36 years old. All my teenage I had acne which resulted in scars, open pores &
rough uneven skin tone. As of now I dont get acne much, but I have black marks on my face (especially on nose) which are result of acne (picture attached). I tried several produts for the black marks but they never fade away. Its been years! I also have pigmentation around mouth area.

Please suggest something (internal/external) to remove black marks & pigmentation.

I am also looking for anti aging solution. Is there any regular regime that I can follow to delay the aging process?

Hello,
Thank you for writing to us on HCM.
I have reviewed your clinical picture and its quite clear you are having post-inflammatory hyperpigmentation secondary to your acne and hormonal condition (PCOD).

Since the pigmentation around your mouth is related to your PCOD, its very difficult to obtain very gratifying results. In your case, I would suggest you the following regimen:
Tablet Nia 250 (Niacinamide) twice a day for 2-3 months,
Tab. Limcee (Vitamin C) (to be sucked) once a day
Apply a comination of mometasone and terfinafine(Sebifin-Plus cream) twice a day over dark aread around lips followed by a good moisturiser like Cetaphil DAM.

As for the post inflammatory hyperpigmentation on cheeks, I would recommend you both home-based and clinic-based treatment.
At home you can try using cream containing Liquorice extract, Soy isoflavones kojic acid, tetrahydrocurcumin, arbutin etc (Melaglow New cream) twice a day over full face for atleast for 4 months.
At bed-time apply 4%hydroquinone cream pin-point over dark marks. (Ideally with an ear-bud !)
For clinic-based treatment, I would like do series of glycolic acid, salicycllic acid, retinol peels for optimum results. I am sure peels will bring very good outcome if done in rightful way.

For anti-ageing, of all the options my personal favourite is Restylane Day and Night cream or Avene Ystheal cream. I would advise you to start anti-ageing treatment after your pigmentation is resolved.

Thank you very much for your detailed response!

I also have hair fall, mild dandruff & my hair is thinning day by day. My dandruff is under control only when I use a anti-dandruff shampoo. If I use any other shampoo, dandruff returns. Please recommend a solution for hair fall & dandruff.

My hair is very oily & I wash it every day. Is it ok to wash hair daily?

It seems you are suffering from Seborrheic dermatitis.
Excess sebum(oily material) production in scalp promotes fungal growth and these in turn eventually contributes to a type of dermatitis known as seborrheic dermatitis.
Seborrheic dermatitis can secondarily lead to hairfall and thus its important to treat it first hand.
I would recommend you a short course of oral antifungal drugs like ketoconazole 200mg daily for 10-15 days and a lotion containing clotrimazole and betamethasone (Candid-B lotion) twice a day over scalp for few weeks.
Use a medicated shampoo containing ketoconazole and salicylic acid twice a week. (Salisia-KT shampoo)
Take antihistaminics as required for itching.
Avoid excessive use of hair oil.
As for the hairfall, start oral supplements containing biotin and other essential vitamins and minerals (Follihair new) once a day for atleast 3 months.
