Treatment of dvt depends on many factors like size of clot, site, risk of pulmonary embolism, patient's general conditions etc.
First line of treatment is heparin. It is an anti coagulant drug. It prevents further clotting in blood. Nowadays low molecular weight heparin is available which can be given subcutaneously in abdomen.
Later on warfarin should be given and continued for 6 months along side regular INR monitoring.
Leg elevation and compression stock bandage help the blood flow.
If there is chance of clot detechment and high risk of embolism then thrombolysis of clot can be done by vascular surgeon. They directly give injection in clot.
Vena cava filter can be put to filter any embolus and prevent pulmonary embolism.
Rarely surgical removal of clot is needed.

Clexane contain enoxaparin. It is low molecular weight heparin. So right now if patient is on clexane then no need warfarin. Warfarin is oral drug it should given after you complete the course of heparin. Dosage of warfarin depends on many factors like weight, other drug interaction, your Pt and INR level etc. So before starting warferin you should go for PT with INR test. Then you should start warfarin according to your doctor's advice. It is prescription based medicine so you should start it according to your treating doctor's advice only.
