At age 42 , hormonal imbalance is common. You can very well be perk menopausal . But you should have associated symptoms of menopause apart from irregular prolonged bleeding like hot flushes or decreased libido or mood changes etc . You fibroid however is of significant size and needs intervention . I think that your prolonged bleeding with clots may be due to the fibroid . Management is required in symptomatic fibroids . However fibroid .are known to the shrink after menopause . But menopause needs to be confirmed if you wish to withhold treatment .
If you don't have associated menopausal symptoms it's possible that the fibroid is causing the bleeding . Fibroids can be treated medically or surgically . You are already taking medical treatment . However in my opinion fibroids larger than 5cm need surgical management .
