Posted on Thu, 16 Nov 2017
Question: My wife diagnosed with dengue on 22nd Oct (4th day of fever). From yesterday, her temperature is returned to normal. but she is getting rashes all over and painfully itchy in palm and feet. She could not sleep the whole night yesterday. Today she discharged 2-3 drops of blood though her menstruation period got over last week. CBC is done daily and today we have done the LFT test. She is drinking plenty of liquid (around 3-4ltr). for fever she was taking dolo 650 from last 1 week. lft test shows sgot 66 and sgpt 57. She is feeling the need to go to bathroom frequently (for both urinating and stool). Platelet count is decreasing consistently and as of today 1.39 lakh. pls suggest what to do? Are enzyme levels suggest any warning? Attaching all the reports from last 3 days.

Uploaded the reports.
About urination: there is no pain. But whenever she drinks water, she gets the urge after some time. Approximately 10-12 times today. Her bowel movement also very frequent. Urine volume is more like her bladder gets full. She drinks around 4 ltr water after being diagnosed. Physically, itching is the main problem right now. Can we do anything about itching. We tried coconut oil and calamine lotion. She feels weak as well.

Brief Answer:
See below!

Detailed Answer:
Hi,

1. The reduced platelets are consistent with her diagnosis. Unless she starts bleeding from mouth, nostrils, etc, there is no reason to panic about this. The numbers will rise progressively.

2. Her increased urination and stools can be attributed to the increased fluids intake. I will encourage her to step down to 3liters daily and monitor her urination pattern and stools.

3. Add Zyrtec for itches. This will improve her quality of life.

I hope these guide and help. I wish your wife well. Feel free to follow up with me if need be.

Thank you so much. Last night she took Allegra 120. Today she is feeling much better. No fever. Rashes also reduced a lot. Itching is there but very manageable.
Only one more question though. What about her SGOT(66) and SGPT(57) numbers? Should I be doing anything about it? GP suggested udiliv 150. But I am a bit apprehensive about taking anything regarding liver. Pls suggest.

Brief Answer:
Control the liver test in 2months!

Detailed Answer:
Hi,

I will not add on this at this time. I believe it is part of the bigger picture. We should control the laboratory tests in 1month. If they are still abnormal, then I will recommend we see a gastroenterologist for further evaluation.

I wish you well and keep me updated!
