Suggest Treatment For Depression And Fatigue When Suffering From Hypothyroidism
Question: Hello Doctor, 4 months ago, I was diagonosed with hypothyroidism and have been taking synthroid 50mcg then. I am 31 years old male. I've not been feeling good since 4 months..taking this medicine is making any good. Before was diagonosed, my TSH was 5.37 (0.4 to 4.40 range). Both T4 and T3 were in normal range. I get tired very easily and feel shortness of breath. My social life is almost nil. I can't talk and walk frequently like I used to be. I've till now have gone thru 30 blood tests..LDL cholestrol level is normal. However, HDL is 32 (> 40), a bit low. I'm feeling very depressed, low in energy level, irregular heart beats. Please suggest me how would I go from here so that I feel healthy again..it is affecting office works as well..I feel afraid of going to office thinking I've to walk and talk much..

First of all you don't require treatment for Hypothyroidism as your TSH Level is in range of Subclinical Hypothyroidism which we don't treat. We just monitor TSH levels every 3 months.
I would advise you to stop thyroid medicines.
Get following investigations to rule out other causes for your problems:
1. Hemogram
2. Liver and Kidney function test.
3. Fasting Blood sugar.
4. Serum Calcium.
5. ECG
Start on supplementation of Vitamin D3.

6. in July, test blood works for TSH, T4
T4, FREE 1.3 ng/dL (0.8-1.8 )
7. TESTOSTERONE, TOTAL, LC/MS/MS - 494 ng/dL (ng/dL (250-1100 )
8. T4, FREE 1.3 ng/dL (0.8-1.8 )
9. my Thyroid peroxide antibodies was 218 (<35)..it is high. My last test on this was on in XXXXXXX
10. Echo 2D Color Flow - came good
11. Holter Monitor - shows irregular heart beats
12. Have experienced infertility as well..We're trying to have a baby since 2 yrs..but I could not get my wife pregnant..my recent two semen analysis shows 0 sperm count..FSH is 8.9 ( 1 to 8)
13. LH count is in normal range
14. Glucose is 82, no diabetes
15 also, my RBC count is quite high 6.5(<5.8) with irregular levels of MCV, MCH, RDW, etc..
16. No anemia issues..HCG count is good
17. No thalasemia issues
18. Doctor diagnosed me with Hashmito's disease
Currently taking 50 MCG synthorid, 1000 IU Vitamin D3 and Maca 500mcg sometimes, Herbal vitamins sometimes..
Not sure about my Iodine and Selenium levels..multivitamin that I am taking sometimes contains 75mcg selenium and 75 mcg.
Please suggest..how to solve my fatigue, shortness of breath..irregular heart beats, body aches(when i do a bit of walking, my body severely ache during night times)

Since your TPO antibodies were high, you definitely require treatment for thyroid. So continue with the medications.
Now irregular heart beat is an important concern at this age.
I would suggest you two things:
1. Holter Monitoring
2. Echo cardiography.
3. Anti Nuclear Antibodies (ANA)
4. Rheumatoid Factor , ESR.
5. Vitamin B12 levels.
Start on a supplement containing Levocarnitine like Neurokind-LC.
Continue with the other medications as you have been advised.

thanks for the response..have one quick question...will these TPO or Hashimoto's thyroid disorders have an impact on my FSH or fertility? I was thinking if this can be treated, my FSH will be in normal range again. I've an appointment with an urology on this issue, I will update you once this is done. Thanks, XXXX

Hi -
I've one more quick question..I'm low in Vitamin D when I did tested it in XXXXXXX was 21 ( 30-10) which is insufficient and I think this has an effect on increase of TPO in my body. I would like to know if I want to bring my Vitamin D3 level to 50-60, how much should I take the D3 supplement?

I don't think that is significant.
Still, there is no harm in taking Vitamin D supplement.
