Suggest Treatment For Depression And Insecurity Feeling

Myself XXXXX, Assistant Professor at University and my brother a consultant settled in Australia. The problem is with my younger sister, XXXXX, aged 18. She is very shy, introvert, has very low self confidence, performs poorly in study, has become big addict of television. Hates meeting or facing people, has no friends, neither goes out for playing, if forced to go to a party sits quietly at one place, eats her meal and comes back. She remains silent whole day, hardly utters 5-10 words in whole day. Seems gloomy and lost.

She is 10 years younger to me and 12 years younger to my brother. I once asked her if she has any personal problem or if anyone is troubling her in the school. She said no to all this, then I asked her the reason for such behavior, she said she remains conscious about her teeth, she has uneven disfigured teeth and each times she laughs she keeps her hand over her mouth. I told her this is not a big issue, we will get that treated. I am confused if this is the only reason behind all her symptoms or something else. She also told me that she hates when family members compare her with us or family cousins and want her to score as they do. I think we too treat her as if she can’t do anything. Whenever any guests come to our house, she is a matter of discussions like, kitna tv dekhti hai, bahar kyu nahi jati, aur bachho ko dekho, padhti nahi hai, fail ho jayegi, party me XXXXXXX XXXXXXX dost bana. But yes she is over pampered and everyone loves her a lot except for this constant nagging. She used to be ok till 8th class (although she never was extrovert or expressive, but was happy in her own world) but these changes came from 9th class onwards. We changed her school and shifted her to DAV school which had a good number of students. We are planning to send her to boarding college. I am worried how she’ll cope up outside. What if her insecurity worsened her life and career? Kindly help.

I have read the query carefully and I think this is not just a case of depression or insecurity..it is psychosis. She behaves abnormally- like : sitting alone all time, hardly speaks..gives undue attention to her teeth.. she watches TV all time and avoids human interaction..she has almost formed an opinion that people hate her/ dont care forher - which is wrong.

You have not mentioned why but you have never taken her to a psychiatrist..she definitely needs one.
You should have gone beyond and inquired about her feelings about her looks and teeth in detail..saying that they can be treated by a dentist - gave an end to all conversation about her ongoing thoughts. We have to know about a persons thought process in order to make a diagnosis- why a person behaves in the way he does and what are his own thoughs about particular belief he has..

Dont worry, many patients in psychiatry are like this, but there are several tests which help in making a diagnosis.

Tell me if she -
1. remains suspicious towards family memebers or memeber
2. ever seen her making silly faces without any relevant reasons or looking herself at mirror for long time when alone
3. or self.smiling or self muttering when alone
4. getting angry fast
5. her sleep pattern
There are many details left which need to be looked into.

P.S. and most important is - donot send her to hostel till her treatment is started or atleast we know what is going on.
