Question: Hi, I suffer from major depression and psychosis with negative symptoms. At the moment my GP has me taking 40 mg of fluoxetine and 100mg of quetiapine in the morning, and can take a extra 25-50 mg of quetiapine during the day, and he also has me on 300 mg of quetiapine at night, and I can also take one tablet of 5mg of diazepam up to three times a day when required. I have been feeling very depressed for a number of weeks recently. I've had my mental health conditions since 2008, and would be very greatful for your advice to help me get better.

I can understand your concern. Depressive disorders follow chronic course and in depression with psychotic symptoms high doses of medicines are needed to get best results.

You are on high dose of Quetiapine which is a good antipsychotic drug with good efficacy in mood stabilisation too. But in depressive symptoms or negative symptoms the drug has limited efficacy. If your psychotic symptoms are under control then one option is to reduce the dose of Quetiapine to about 200 mg in evening which will help in sleep and to add other drugs like Amisulpiride. Amisulpiride is also an antipsychotic but great efficacy in depressive disorders and negatives symptoms. The drug in low doses have good antidepressant effect. You can talk with your psychiatrist for this drug.

As you are on Fluoxetine 40 and symptoms are still persisting so this is the time to increase the dose. Increasing the dose of Fluoxetine along with addition of Amisulpiride should help you in resistant symptoms.

So please discuss with your psychiatrist for the drug changes I've mentioned and your symptoms should improve.
