im having too many hi and lows,but nurse does not change my meds

Depression is very much a treatable disease. You need to apprise your treating psychiatrist about the situation. XXXXXXX mental state exam and a detailed sit in history would be required to devise a plan with respect to adding or replacing further medication. Sertraline is an SSRI which has a better cardiac safety profile and can be considered for switching with Effexor which you are already using. Deep breathing exercises, ensuring at least 6 to 8 hours sleep daily and exercise for 30 minutes regularly. Behavioral cognitive therapy and psychotherapy sessions should be utilized. The therapist can guide you towards tailoring daily routines that would be helpful in coping with the issues that you face. If you have any suicidal thoughts then you should go to an ER near you immediately.
