Suggest Treatment For Difficulty In Conceiving Despite Going Through Infertility Treatment

Question: Hi I am XXXXXXX age 31 female suffering from pcos
Till 2012 I was on and off on fertility treatment without success
Now the problem is I always ve heavy periods 7 to 8 days
2015 Feb I had normal periods after that I did not had periods ( I will not have periods on my own due to hormonal imabalance)
So this month took regestrone for 10 days after stopping pills after 4 days I got my periods first day it was very scanty from 2nd to 4 th days it was very heavy ( 4 - 5 pads) today is my 7 th day I'm still bleeding but not heavy
I'm very obese 176 ht _ 125wt in order to regularise my periods startrd ocp yaz from day 1 of my period
From 2009 to 2012 I ve been numerous ultrasound scan everything was normal except pcos
Pap was normal
Now my concern is prolonged periods
Is this happening because of having periods after so long or anything to concern about
When I chk in Internet all topics are related to cancer I'm scared

I understand your anxiety related to the issue.
It is definitely not normal to have such a period pattern and especially when you have history of PCOD and infertility for a long time along with obesity.
So in my opinion you need to get an endometrial biopsy done so as to get the proper detailed evaluation of the endometrial health.
In the meanwhile I would also like you to get an ultrasound via transvaginal route before biopsy to see the endometrial thickness and ovarian size.
Get it done and then we will discuss further management.

Yes mam, there is a definite chance of endometrial hyperplasia as -
- you are overweight
- high society life style
- history of PCOD and infertility
- irregular period pattern and erratic bleeding pattern.
All these are risk factors and moreover any irregular bleeding pattern needs evaluation.
Also I would like you to get your thyroid hormone levels and prolactin levels along with blood sugar levels along with transvaginal ultrasound.
This will complete your work up and if normal will be reassuring.
