After reviewing your case history I'm of the following opinion:
1. Diffuse idiopathic skeletal hyperkeratosis (DISH) although is a non-inflammatory condition which may cause no symptoms, so let me know whether presently you are symptomatic (pain/stiffness) or asymptomatic?
2. Since it is present in 19% of men over 50 years of age, so could be age-related.
3. Since there is a close relationship between DISH and obesity- insulin resistance and diabetes: are you having any associated risk factor?
Do revert back with your concern so that further dietary regime and lifestyle management can be initiated.

Thanks for reverting back
1. Since the said pathology DISH mainly affects the back and causes stiffness of the thoracic and lumbar spine [causing inflammation of deep muscles of the spine and its ligament] thus probably explains your symptoms.
PS. Limited range of motion and pain in neck and difficulty with swallowing indicates that ligament of the neck are affected and disease is progressing and because you are overweight that too is contributing to the pathogenesis.
2. Make sure to consult with a Rheumatologist to exclude any seronegative Spondyloarthropathy, since it can occur with other conditions like osteoarthritis and AS [ankylosing spondylitis] which also affect the spine and the entheses.
3. In Ayurveda, we consider Rheumatism/Arthropathy under ‘AMA’ i.e. disease arising from/due to improper metabolism so treatment and Management are directed towards
My suggestions are:
i. Expelling the already formed AMA, methods to prevent further AAM formation [deepan/pachan ]
ii. Avoid urad dal, curd, fish, jaggery, milk, incompatible and heavy food, suppression of natural urges, anxiety, grief, outdoor activities when the sky is covered with clouds, and cold wind.
iii. Take maize, horse gram, bitter gourd, dried ginger, garlic, ajwain, saunf, black pepper, saindhav lavan [rock salt], asafoetida, cumin, old ghee, lukewarm water, castor oil, light walking, sudation, warm clothes, garlic and ginger mixed buttermilk.
iv. Fry 250 ml ginger juice in 250 ml of til tail, when 250 ml remains [make sure oil doesn't get burn], filter it after cooling and massage.
v. Boil 1 spoon dried ginger in 4 cups of water till 1 cup remains, filter it and add 2 spoon castor oil and drink.

PS. since as you already know that Immunosuppressants is/are required for treatment purpose and the majority of disease is AI [autoimmune] in nature, as such I will start with Rasayana chikitsa [Immunomodulation] therapy in you.
5. As you are overweight thus following guidelines are worth mentioning:
i. Half an hour of aerobic exercise. [running/jogging/swimming/cycling
ii. Avoid sedentary lifestyle and develop healthy eating habits such as taking plenty of salads prepared with green vegetables.
6. Take care of your spine.
i. Relax the spine completely.
ii. Do not lie face down.
iii. Lie on your back with a pillow under the knee.
iv. Lie on the side with a pillow between the knees.
PS. Too much straining during defecation can cause sprain in the muscles of the back, make sure you are not suffering from constipation
