History of thyroid removals urgery and use of thyroxine of just 175 mcg may not be adequate specially with simultaneous use of diuretics where clearance is increased. Get thyroid hormones evaluation and consider increasing the dose till you achieve euthyroid status. All you symptoms will fade including dizziness.
The medication both lisinopril and triamterene and hct in combination causes this dizziness as side effect as well. You may consider symptomatic treatment with antivert etc.
Swollen feet should be evaluated for cardiac problem as congestive heart failure or renal functions derangement both can lead to pedal edema.
