Brief Answer:
Need to rule out brain ischemia.

Detailed Answer:
Hi,

Thank you for posting your query.

I have noted your symptoms.

Based on those, we need to exclude a diagnosis of brain ischemia (lack of blood supply to brain). This can be done by doing an MRI of brain.

For relief of symptoms until then, you may take symptomatic treatment (betahistine for dizziness and diclofenac for pain).

I hope my answer helps. Please get back if you have any follow up queries or if you require any additional information.

Wishing you good health,

Nine years ago I had an MRI which showed low density .33 rt. frontal periventricular whate matter wo other evidence of chronic ischemic change. The Cscan I recently took showed the matter to be l.9 no ischemic concens or hemmorage. I tried the epley manuever which seemed to help the dizziness somewhat, also failed two balance tests a few days ago -- walking with eyes closed -- felt unbalanced and marching in place with eyes closed - when I opened my eyes, I had turned to the left.

Brief Answer:
MRI is preferable.

Detailed Answer:
Thank you for getting back.

It is good to note that you are better with Epley's maneuver. This could favor a diagnosis of BPPV- benign paroxysmal positional vertigo. An ENT re-evaluation would be helpful.

MRI is preferred over CAT scan, as small ischemic changes are better seen on the MRI.

Best wishes,

Dr Sudhir Kumar MD DM (Neurology)
