Question: hi doc , yesterday i was bit by my pet dog and its a bit deep injury it is vaccinated from time to time and its vacsinated 4 months ago......i went to the hospital and was given a dose of TT injection, and rabipur and diclofinac injections.... but the problem here is at the time my dog bite me i was drunken drank nearly 400 ml of scotch wiskey...... i just wanted to know if the injections really work as i was drunken pls help me out its been 16 hrs since the bite

Brief Answer: Take one extra dose of Rabipur Detailed Answer: Hello, Welcome to Healthcare Magic, As you were bit by your pet dog which was vaccinated up to date against the disease rabies. Following the dog bite you have taken a dose of anti rabies vaccine (Inj Rabipur), TT injection and Inj Diclofenac. As you have boozed alcohol at the time of the dog bite, You should have taken two dose of antirabies vaccine on to two different shoulder. This is because alcohol will suppress the immune system and it may delay the induction of protective antibody titres from the immune system against the disease rabies. So i would suggest you to take extra one more dose of antirabies vaccine (Inj Rabipur) on to the other shoulder as early as possible. Then you have to complete the full schedule of vaccination i.e Inj rabipur on days 0,3,7,14 and 28. Day 0 is the day of first shot of vaccination, here you need to take one more shot of Rabipur injection before the second shot of Rabipur. Inj Rabipur should be taken on to the shoulder by intramuscular route. If your dog is healthy after three shots of inj Rabipur, you can discontinue the remaining two shots of injection. but I also drank 3 shots of scotch today so is it fine if I take the rabipur one injection and what about the tt injection should I take it once more

Brief Answer: No need to take TT injection Detailed Answer: Hello, The disease rabies is 100% fatal but it is 100% preventable by proper and adequate treatment. During the treatment you should not take alcohol, as I explained earlier it is going to delay the induction of Rabies virus neutralizing antibody (RVnAb), which is very essential to protect you against getting rabies. If you have taken alcohol today tomorrow you can go and take a extra dose of antirabies vaccine and you should not booze until the completion of all the doses of anti rabies vaccine. Regarding TT injection, there is no need to take extra dose of TT injection your body will be boosted to protect you against the development of tetanus. Brief Answer: Schedule is proper Detailed Answer: Hello, There is no discrepancy in the schedule, post exposure prophylaxis schedule remains the same as I have explained earlier that you have to take vaccine on days 0,3,7,14 and 28. As you have consume more alcohol on the day of bite according WHO guideline, you have to take one extra dose on day 0. As already 16 hours has elapsed following the bite still you can take one more dose before the scheduled second dose. I hope this has cleared all your doubts regarding your treatment.
