Suggest Treatment For Dull Pain In The Left Testicle

Question: I have been having dull pain in my left testicle for last 2-3 weeks and I reached out to a doctor here for that.
My physical examination was done and urinalysis was ordered.
Symptom wise- apart from above mentioned dull pain, I have no issues. No fever, body pain etc.
I was diagnosed with Prostatits and with that I was asked to start 'Cipro' (ciprofloxacin) 500 mg, 2 tablets daily for 10 days.

I have taken 1 day's dose of the medicine, but I wanted to check if it's a good idea to go for the full 10 day course, if urinalysis doesn't point to any infection.

Also, is Cipro safe, are there any side effects of the above antibiotic that i should be watching out for?

Based on the facts that you have posted the dull aching pain in left testicle can be due to one of the following few factors
1) Infection of Epididymis (Epididymitis) or Spermatic cord (Funicullitis)
2) Varicocele
3) Mild infection of Testicle itself (Orchitis)

I shall be happy if you can provide me few more details to make precise comments and suggestions on your problem .
1) Do you have any urinary symptoms like burning ,pain ?
2) Do you have pain while ejaculating ?
3) Do you have pain in perineum or around anal region ?
4) Do you have discomfort in lower abdomen ?,

Since Prostatitis needs to be treated with appropriate antibiotics for 4-6 weeks The diagnosis of Prostatitis has to be confirmed by clinical and Digital Rectal examination by a qualified Urologist and following basic tests
1) Urine culture
2) Ultrasound Scanning of abdomen and Pelvis
3) Ultrasound Scanning of Scrotum

Hence I would suggest you to consult qualified Urologist for detail clinical assessment to confirm the diagnosis before taking any antibiotics .

Further treatment will depend upon result of these tests and final diagnosis.

Hi Doc,
Thanks for your reply.
As for the symptoms-
I don't have any burning sensation while urinating
No pain while ejaculating
No pain around anal region
No noticeable lower back pain. I have mild stiffness in lower back due to the nature of my work (software), but this stiffness has been there for some time and is manageable.

Also, I'm based out of USA, right now and went to a primary care physician here. I've already started on Cipro.
I'm waiting for my Urinalysis results to come back (i will share them with you as soon as I get it).
Now that I've already started on Antibiotic, should I continue it for the prescribed 10 days (500 mg, 2 tablets daily) or should I reduce the dosage till i don't get my Urinalysis results? Because, i think, i should not to be stopping Antibiotic mid-way.

I was told by the Primary care physician that if my pain doesn't subside after 10 days of antibiotics then he will go for ultrasound. Should I wait that long? or I directly go to a Urologist as per your suggestion?

Based on your feed back I do not think that you have Prostatitis .However Ultrasound Scanning of Scrotum will help to pin point diagnosis .

You can continue taking Cipro for a week and take a call on for consultation with Urologist depending upon the response to Cipro .I would suggest you to take anti inflammatory drug like Diclofenac twice daily along with Cipro .

Is there any over the counter medicine that you know of, available in USA that has the same ingredient as 'Diclofenac' or do I need to get a prescription for that?
I might not be able to get a medicine without prescription here :-)

I've got my Urinalysis results (Urine culture is still awaited), is there a way to upload those records so that you can have a look please.Hi Doc,
I was able to figure out the upload.
I've uploaded 2 documents- One is my doctor visit summary and the other is Urinalysis that i got today.
Urine culture is awaited.
Is it ok to reduce the Cipro dosage to 1 tab 500 mg (instead of 2 tablets/day) for the next couple of days as I wait for my Urine culture?
If Urine culture is negative then it will probably stop the Antibiotic or go back to 2 tabs if the culture shows some issues.

In absence of any positive evidence and the relevat tsts being not done I do not agree with diagnosis of Prostatitis .

Taking half dose of Cipro does not serve any purpose Take twice daily .It will help to combat infection if at all it is there .

Get Ultrasound Scanning suggested in my earlier reply done to come to definite conclusion

Hi Doc.
I had a consultation with a Urologist and he diagnosed me with Epididymitis. But based on my urine culture (normal) and urine analysis (normal), he didn't advise me to take any antibiotic. He asked me to take over the counter Ibruprofen (as you had suggested) to bring down the inflammation. He told that there is a cyst but over the testicles and it seems to be benign but i might have hurt/irritated it while doing exercises (i do treadmill and elliptical in gym daily). He asked me to go slow for some time and let the inflammation come down.
The doc also asked me to go for a ultrasound of abdomen and scortum to rule out kidney stone (i also have a little high uric acid- around 7).
The ultrasound will be probably done in a week's time.
I will update you with the results.

As regards cyst in Epididymis it is always benign in nature and does not reqquire any active intervention .
