I think I may have the solution to your problems.
There seems to be two problems you are facing
1. Hypothyroidism. Your TSH is raised. So its likely that your thyroid hormones are not getting secreted appropriately. The solution is to start Thyroid replacement (Thyronorm.Eltroxin 25) once daily on empty stomach. This medicine will need to be continued regularly. And repeat thyroid function tests in 3 months. (The dose may be different if you are already on the medication for thyroid). The dose may be adjusted thereafter as per the report.
2. Cervical spondylosis: Your headache, shoulder pain and back pain sound like the effects of spondylosis. I will suggest the x rays of the cervical spine and lumboasacral spine. These will help us confirm the diagnosis. If we get a positive result, a prescription of pregabalin and methylcobalamin XXXXXXX be needed along with effective physiotherapy.
