Medications Tried: gave the 15 units to lower sugar retested at 15 still high....

my mother blood sugar just shot up to 400...she just got out of the hospital as she has been fighting a UTI for 2 months...they did hydration and iV antibotics....I wanted she to go to a rehab hospital as she is recovering from a broken hip and shoulder and now two months out they are allowing her to use and bare some weight on the arm...but they said no and sent her home after 48 hrs...

I just rechecked the sugar and it is 420 I set the timer to recheck in 10 min as she took 15 unit of fast acting about 20 mins ago...she had not missed any doses she took at dinner plus the long acting she takes every night...

Brief Answer:
Needs admission

Detailed Answer:
Hi,
I am Dr Mittal.
I have read your message.
I think I can help you.
The fact remains that her sugars are persistently high.
The fact also remains that sugars do rise like this after infection.
I would like to know other medicines she is on especially steroids which many doctors often use in infections and which is known to raise sugars.

In any case such high sugars need testing for urine ketone and Arterial blood gas also.
Also, it's better to admit her for sugar control.
She will need intravenous insulin infusion. Along with other measures like iv fluids.

As for now, reduce sugar charting to hourly and give insulin accordingly. This is because sometimes their body accumulates insulin in skin and then suddenly sugars drop when insulin comes into circulation.
Frankly , even that needs hospitalisation. So I think you should meet your physician / diabetic specialist for admission and iv insulin.

I hope that the information contained in this message will be able to help you find the best solution for your problem.
Feel free to contact me for further clarification as needed.
Best of luck. Dr Mittal.
