Suggest Treatment For Elevated Neuron Specific Enolase

Question: I have elevated neuron specific enolase in revent labs. Speckled ANA, elevated aldosterone, but normal DHEAS. I have normochromic, normocytic anemia woth increased retics and low serum iron. Total ferritin is normal. Swollen non tender lymph nodes, enlarged parotid ans peripheral edema with enlarged abdomen.Ive taken remicade and then humira since 2003. What dr should be helping me thru this?

Thanks for your query.

It appears that you have some kind of connective tissue disorder (autoimmune). That can explain most of the findings. I also note that you have taken two medicines for autoimmune disorder for >10 years.

So I think rheumatologist is the best person to guide you. The low serum iron with anemia needs to be treated, preferably by a hematologist. Elevated aldosterone may not need any treatment unless you are having lot of fluid retention or low potassium in blood. Neuron specific enolase is a non-specific finding. Though it is elevated in some tumors as well, in your case probably this is non-specific. Of course I would do routine Xrays of chest and ultrasound abdomen to rule out any tumor.
