I would strongly suggest to bring him to the ER where a physical examination and ultrasound can be done to determine the sudden enlarged scrotum. The most dreaded condition that we haveto rule out is a strangulated hernia which prompts emergency operation. Other condition that is not so urgent would be hydrocoele (water in the testis) and edema but this would be generalized and not localized to the testis.

The enlargement appears to be in the left testicle area only, not the entire scrotum.

If it's on one side only it means it's not generalized edema but could still be hernia or hydrocoele

What can you tell me about them?
I'm told they are often caused by bacteria, and an anti-biotic was prescribed, Ceftin.

hydrocoele ? it is water collection in the the testis. it could be secondary to infection (UTI). if it is recurrent and didn't resolve after antibiotic then surgery might be needed to excise the fluid and the capsule that traps the fluid.
