Suggest Treatment For Enlarged Mediastinal Lymph Nodes In Chest

Question: Hi I have a number of mediastinial lymph nodes in my chest that are enlarged. A following pet scan showed intense activity in this area and also in my adenoids and tonsils and the ano-rectal area. There was also Moderate activity in the bones. They removed and biopsied one tonsil but it came back as normal except for tonsilitis (I wasnt even sick or unwell). They just did a bone marrow biopsy - also normal. All blood tests have been normal - including ldh and flow cytometry - all normal except for Beta 2 microglobulin which was 2.2 on 10th dec (normal level I am told here is 2.0) and XXXXXXX to 2.6 on 31st dec. I have been incredibly tired this year and have had problems taking a deep breath, like my chest is tight, for 6 months or so. I am very hot at night to the point where I sleep with a fan on most nights to keep me cool. I itch all the time but im not sure if this is psoriasis which Ive had before or dry skin due to fan on all night. And I often get quite achy - like im about to get sick. I have had Lungs looked at on scans and lung function test, spirometry - all were normal. I am having a colonoscopy next week to query the activity on the pet scan. I have had issues in this regard that I had previously mentioned to my GP, sudden intermittent constipation and diahhorea, loud bowel noises etc. My doctor is now telling me the next step is to biopsy one of the enlarged lymph nodes by going in between two of my ribs. All of the talk has been around lymphoma with the possibility of sarcoidosis, however a blood test they ran that might indicate sarcoidosis was negative. Is there a possibility that this could be lymphoma with all blood tests except the beta 2 essentially normal?

It is still remotely possible that this could represent lymphoma or possibly sarcoidosis. A biopsy will be necessary to provide a definitve diagnosis as you know.

At the same time this may not necessarily be either of these two things and may represent an inflammatory process.

Your symptoms of fatigue and feeling very warm at night are suspicious but again do not tell us for sure what is going on.

I can appreciate that you are dealing with some stress and anxiety related to all of this. I think a biopsy of the hyperactive lymph nodes represents a logical way to approach this issue.

One test which I wonder if was done is the LDH - a blood test. Do you recall if this was done?

Yes this was done and I was told it showed nothing of concern. The only real blood results that showed anything was the beta 2 micro globulin that was elevated initially and then more elevated on retesting 3 weeks later.

These test results seem to weigh more against a lymphoma. A biopsy is still very reasonable however and I think you should review that with your primary doctor.
