Suggest Treatment For Enlarged Prostate, UTI And Burning Sensation During Urination

Question: Dear Doc. I had enlarged prostate, about 60 mm and PSA 15.5 (tested locally). I was given medicine Silofast-D which I took for sometime. I went to Apollo, New XXXXXXX and repeat PSA retested was Free 0.32 and Total 1.2 + Uroflowmetry Normal. Doctor told me I hade UTI. He prescribed Niftran 100 mg twice daily & Flotral 10 mg once daily for six weeks. During the period I was taking medicine, I felt ok. But once I completed the course and discontinued medicine, I have started feeling frequently some burning sensation (something like acidic) while urinating. I could not get further advice from my Doc. as he seemed to have gone on leave/holidays. Kindly guide me.

Dear Doctor,
Thanks. Here is detail you have kindly asked :
My Age : 67
High blood pressure : No
Diabetes : No
Any Other Ailment : No
March, 2015
Urinary Problem started.
I was taking longer to empty the bladder & some burning sensation + some discomfort.
Locally checked PAS (about 15) and Prostrate size (about 60mm).
Local Doctor prescribed : Silofast-D which I took for about 10-15 days
During this period I felt ok.
April, 2015
Second Opinion at Apollo (ND) : repeat tested : PSA Free 0.32 and Total 1.2 + Uroflowmetry Normal. Prostate size : not tested.
Doctor diagnosed : UTI
He prescribed Niftran 100 mg twice daily + Flotral 10 mg once daily for six weeks.
Course completed 5th XXXXXXX 15
After stoppage of medicine I experienced frequent slight burning sensations and discomfort
Original Doctor in ND could not contacted. Substitute doctor advised continuance of Nifran & Flotral for a week.
I have re-started above medicine.
Seems it is helping.
My question:
Should I continue above medicine for more than 1 week so that I get rid of UTI which I feel is quite heavy.
Even should I take Niftran 3 times in 24 hours in place of 2 times ?
If so, how long should I take above medicines ?
I also want to retest my PAS and prostrate size. Please advise.
Which medication is better flotral or Silofast-D
Thank you.

Brief Answer:
Please consider these explanation
Detailed Answer:
Hello
Thanks for your reply and detailed description
Looking at your detailed history there could be a possibility of chronic UTI with niftran resistant bacteria
I would advise you for urine culture and sensitivity test and according to the report a course of antibiotics under supervision of your doctor
In majority of cases infection subsides with course of antibiotics for 3 weeks duration
You should not take niftran 3 times in a day it's standard dose is 2 times a day.
You should not take any medicines without doctors advise.
Retest for PSA and prostate is not required
Flotral and silofast D both has different mechanism of action and indicated for prostate hyperplasia
Hope this answers your question
Please write back to me if anything is not clear.
wish you good health
