The enlargement is due to collection of fluid (odema). When hernia is reduced and repaired that area becomes empty space. Surrounding lining secretes fluid causing it to enlarge .
In hernia surgery they won't do anything with testis. Nor they fix it or they remove it. They just place it back into scrotum after repairing hernia. They only remove testis if it is not functioning or in aged (>70yrs) people with recurrent hernia. Before removing they will take permission of patient.
Yes it is not ischemia. It will cause severe pain. For this you need to take anti odema measures like take serratiopeptidase tablets thrice daily and ware tight underware so that it support the scrotum well and reduce odema. Scrotum shouldn't be hanging loosely.
