To recapitulate: Male/42 - epididymis 3 months ago - cyst left - growing gradually - quarter the size of the testicle - wants to know - how common - seeking treatment - expectations of the tests and/ surgery.
It is not much common to develop cyst after epididymitis.
You should seek proper diagnosis - this is important to decide the mode of treatment.
I would advise the following and also note the possibilities you have asked for:
- Clinical evaluation and examination by a General Surgeon. A transillumination tests will indicate whether this is a cyst/secondary hydrocele or something else.
- Ultrasonography with color doppler mode helps to get a confirmed diagnosis.
If there is a secondary hydrocele, you can have wait and watch policy, go for review tests and get operated if indicated so.
It is very difficult to predict and tell you about other possibilities and can just add to either the confusion or apprehension.

Thanks for your prompt reply. I am scheduled for a doctor's visit this week. Could blockage of the epididymis be present after an epididymal infection? Could this cause the mass I currently feel? It is semi soft and somewhat 'squishy' to the touch and feels as if it is filled with fluid. It is also palpable apart from the testicle and definitely feels as if rises from the epididymis.

This looks to be a cystic mass.
As we discussed above, it can be a secondary hydrocele as a reaction to epididymitis, cyst coming from epididymis in this way is not common.
Also get an ultrasonography and color doppler examination (CD) to get a confirmed diagnosis along with clinical evaluation by your Doctor (I hope this is Urologist or General Surgeon)
