Suggest Treatment For Erection Problem And Penis Curvature Post Injury

Question: Hello. I am 52, my partner is 56. About six months ago we were having vigorous sex in the "reverse cowgirl" position. He hollered out and pushed me off and we immediately stopped having intercourse. For weeks afterwards he avoided me sexually although we had previously been active 1 - 2 times daily for the past 3 years. When we did engage, it was after consuming alcohol, in the dark and he would abruptly enter me from behind. Needless to say I was hurt, confused and concerned. Two months ago he admitted that we had fractured his penis. He showed me what was now a very shortened and extremely curved erect penis (almost 90 degrees!). At my urging, he saw a urologist but without me. He told me that he was told there was nothing that could be done and that we just need to get creative. He has since purchased a sex toy that is a very large dildo that slips over his penis like a glove so that the oversized glans and partial shaft is what we have used for penetration. I am uncomfortable with this for many reasons, the main one being that our love life - and subsequently our relationship - is strained and uncomfortable. He seems angry and ashamed, does not want to discuss further medical consultation and likens this to as if he had had prostate surgery and we would need to get creative there too. I disagree. I have read up on it and most suggestions are for immediate surgery. As this is no longer an option, I have also read that there are apparatus that work as a splint during the healing process. Perhaps it is too late for this too. I am less concerned about the physical aspect although I do sincerely miss being comfortable with our bodies again but wonder what if anything I can present to him in a non confrontational manner as a solution besides weird toys and weirder communication. Thank you, XXXXXXX

Brief Answer:
He needs to consult qualified Urologist for evaluation

Detailed Answer:
Hello
Thanks for your query ,based on the facts that you have posted it appears that your partner has sustained injury to his hard erected penis while you were engaged in sex in cowgirl position .

Based on what you have described and subsequent complications like loos of erection and curvature of the penis there is no doubt that he has sustained fracture of penis which should have been treated immediately .

Tunica Albugenia a sheath that envelopes two erectile bodies Corpora Cavernosa has scarred causing curvature of the penis.

He needs to consult qualified Urologist for clinical examination and get following basic test done to confirm the diagnosis .
1) Ultrasound Scanning of Penis
2) Color Doppler Scan of penis to evaluate status of arterial blood flow to penis or any venous leak .

Further treatment will depend upon result of these tests and final diagnosis.

In a present situation there are two options of treatment .
1) If the blood flow to penis is normal and there is no evidence to suggest any venous leak correction of curvature of penis by surgery of what is called as Placation of Tunica Albugenia .

2) If there is damage to erectile tissue and his ED is detected to be Vasculogenic in origin ,the surgery of putting penile prosthesis implant is the best option .This will help both of you to have enjoyable sex for rest of your life .

This is very sentimental issue for both of you and have to discuss it with open mind and resolve it amicably ,

Help and counselling by Psychiatrist may help to convince your partner to take decision .

Hope I have answered your query ,please feel free to ask if you have more questions ,I shall be happy to help you
Thanks and Regards.

Hello Dr. XXXXXXX

Thank you for your response.
He did seek the advice of a specialist who recommended waiting 3 months from that time for what ever progression of the damage to complete. Surgery was not suggested. There was discussion of injections to diminish the scar tissue but as it is on the underside, there was concern that this could damage the urethra so also not an option. It has now been three months since that consultation but he is reluctant to go back as "there is nothing to be done".

I have read on the internet that there is surgery but it looks as all of these need to be immediate. He is still able to get an erection but, as mentioned, there is an extreme loss in girth and length and the angle is 90 degrees downwards. Is it too late for surgery?

Thank you, XXXXXXX
