Suggest Treatment For Extreme Fatigue And Joints Pain

Question: I have a wide range of health problems and after teaching computer science classes for 11 years I have had to resign and go on disability. I have extreme fatigue, pain throughout my joints, I am very forgetful and have problems carrying on a conversation, as I forget what I am saying or get stuck trying to find the right words to say. I have Chronic Active Ebstein Barr Virus, CMV, hypothyroidism, Hypopituitary, pulmonary hypertension due to a defect in my heart, tremors, osteoarthritis, memory problems (as stated before) and a whole laundry list of other problems. I need two partial knee replacements and have metal in both my neck and lower back from fusions. My primary Care Physician has been great and was the first doctor to diagnose the Chronic Epstein Barr virus, which probably has the most impact on my life because of the severe fatigue. The other most important health issue is my memory problems and even though I have been placed on medicine for Alzheimers and Palsy, I don't think I have had much of a change in my memory issues. My doctor has admitted that he has just about come to the end of what he can do for me. He wants to refer me to Vanderbilt Medical Center, but isn't sure what kind of doctor I should see first. Where would you suggest that I begin? I am considering "infectious desease said", an autoimmune specialist, or a neurologist. What type of doctor at Vanderbilt do you suggest I begin with?

I read your question carefully and I understand your concern.

The answer to your question is not that straight forward due to the multitude of conditions you have, several of which could be related to your symptoms, memory issues included. Each of those condition might be responsible alone or contribute with the other issues to cause the memory issues and fatigue you describe.

If the memory issue is the primary concern you want to address my choice would be a neurologist. I believe a neurologist would be the specialist of choice in assessing the severe fatigue you describe as well. If anything he could exclude and avoid taking unnecessary medication for Alzheimer's and Parkinson's.
