Suggest Treatment For Fatigue And Joints Pain

Question: Symptoms are fatigue, body pain - muscles & joint pain, headache, intermittent sweating and chill in the night, but the thermometer shows normal temperature ie 98 degree. What can be the issue ?

Hi and thanks for your query.

This may just be a subclinical viral infection. But if it is not get better then we need to rule out conditions like malaria and typhoid. I would suggest that you get tests done like complete blood count, ESR, complete urine analysis, typhidot and Malaria IgG and IgM.

Only after seeing the test reports something can be suggested.

As suggested the the blood test is done and the results shows Typhi "o" positive. It is 1:160. the doctor also prescribed medication for 5 days. Is this test sufficient to confirm Typhoid and start medication or should I go for any other test to confirm it.

If I start the medication, is there any other test to be done at the end to ensure that it is normal or is it just the absence of the symptoms.

Is there any diet or any other specific precautions to be taken.

Hi again

It seems that you have done widal test. Typhidot can be done to confirm if it is an acute infection or it has been there for longtime. Please upload the reports so that I can see them.

10 day antibiotics are started if the symptoms of typhoid are typical alongwith positive widal. You do have some of the symptoms, though typically typhoid produces diarhea, pain in abdomen, fatigue and fever. Do you have diarhea and pain in abdomen?

Widal can be repeated after the antibiotics. Serial readings of widal on different occasions are undertaken to check the status of infection i.e whether it is getting cured etc.

In diet, take softer foods until your gut heals. Avoid raw vegetables and fruits completely. Avoid fish. Avoid unhygienic foods. Dont eat outside. Drink cleaner water.

Current symptoms include - abdominal pain, fatigue and nausea, but no fever and diarhea. Attaching the blood reports for your review.

You need antibiotics for 10 days

Hi

This does seem like typhoid. In my opinion you need antibiotics for 10 days. Repeat the test 1 week after completion of antibiotic course.

I.M injections of cetriaxone once a day for 10 days is the standard regime for typhoid. Otherwise oral combination of cefixime and oofloxacin can also be taken.

The abdominal pain and nausea will start showing improvement in 3-4 days of treatment but fatigue can last longer. If you feel like vomiting take medicine like ondansetron.

Dietary precautions are really needed. Avoid the raw foods. Softer foods and fruits can be taken. Since it happens from unclean drinking water, that precaution is really needed.
