Welcome and thanks for your query.
Fatigue and weight gain can be due to hypothyroidism.
The pigmentation, if it is velvety can be acanthosis nigricans, a condition associated with insulin resistance.

I fully understand your confusion.
You have had these tests done in two different labs.
The labs may be using different reagents and hence there may be some minor differences but the large difference in TSH between the two is not acceptable.
However there is no need for a panic. Does she have symptoms of hypothyroidism like cold intolerance, constipation or excess menstrual flow? Is her heart rate low?
If these symptoms are not present there is no reason to worry. If these symptoms are present she may be having hypothyroidism. It would be also good to do Thyroid
antibodies as these are likely to be elevated.
If the dark pigmentation is localised then please upload a picture. If it is generalised darkening , then blood for ACTH also may be checked.
Do get back with your input.

Detailed Answer:
It would be good if you can consult an endocrinologist who will examine her and suggest if any additional test is to be done.
I shall appreciate it if you could let me know the doctor's opinion.
