Suggest Treatment For Fatigue, Vertigo And Lightheadedness

Question: I am 70 yo white male. latest physical normal. loss of hearing in right ear recently, have a hearing aid. occasionally get some dizziness, lack of balance which is attributed to right ear problem. last night felt dizzy, some vertigo, lack of balance when looking to my left; dizziness worse this morning, took bp it was high 160/83, some nausea, but did eat ok, fatigue, but recently had a bad cold which is now better. Not sure what to attribute this sudden increase in dizziness and lack of balance. No stroke symptoms

I appreciate your concern

The subjective complaint of dizziness and vertigo should be differentiated. Dizziness usually indicates lightheadedness and leads to blackouts/ syncope. In case of dizziness , a cardiac evaluation is needed including EKG, Echo, stress test.

If vertigo is there , meaning a sensation of movement when there is none in reality, then it can be due to a CNS cause like a stroke, tumor, Multiple sclerosis or a peripheral cause such as Meniere disease, labyrinthitis.

History and Physical exam looking for nystagmus, tinnitus and presence / absence of focal neurologic deficits can be used to differ between a central and peripheral cause of the vertigo.

The dose of diovan can be adjusted to control the systolic blood pressure though its not as high as to independently lead to these symptoms. Though treatment depends upon the cause , you can use betahistine dihydrochloride for the vertigo episodes , in consult with a neurologist.
