Suggest Treatment For Fatty Liver In A 40 Year Old Male Person

Question: Hi I am 40 years Male, from XXXXXXX these days I am feeling weakness. In morning too, I am not feeling fresh. my ultra sound of Upper Abdomen is Grade III fatty infiltration of Liver. other diagnostics report attached. please advise further based on my reports and above information.

Hi,

I have gone through your medical history carefully and understood your concerns.

I was going through the lab tests you provided, and noted as following:
- there is decrease in hemoglobin, MCV, PCV,,, - suggestive of anemia? we should identify WHY? you have it: low iron levels? (by the way, the iron levels were not included in the tests)
- thyroid function test was normal, excluding thyroid pathologies as possible causes of weakness
- high ALT and ALP levels are due to fatty liver, as long as hepatitis testing was negative

At this point, I'd like to have a few more data regarding your actual symptoms:
- since when you started to feel weakness? if just recently , it might be due to a viral infection (flu/cold) or changes in diet (any specific diet restrictions advised for fatty liver?
- are you going through a stressful situation at: work? home?
- do you drink caffeinated drinks or alcohol especially in the afternoon?
- do you stay longer at night?
- any other symptom apart weakness: pain? fever? diarrhea? etc.?
- any medication taken on regular basis?
- any contact with ill patients (with cold? flu?)

All these data are necessary to understand your complete health problems before running into final conclusions.

Answer: I am feeling low since many months. although I do not have fever, but my hand palm are mostly hot and my eyes burning.
Last year in August 2016, there was Vitamin D was low and doctor was prescribed me some injections. which I could not complete course that time.

- are you going through a stressful situation at: work? home?:
Answer: some time.

- do you drink caffeinated drinks or alcohol especially in the afternoon?
Answer: Yes, we drink milk Tea and coffee mostly 3 times a day.

- do you stay longer at night?
Answer: not regularly.

- any other symptom apart weakness: pain? fever? diarrhea? etc.?
Answer: heart beat some time irregular, joints pain but not major.

- any medication taken on regular basis?
Answer: No, except paracetamol.

- any contact with ill patients (with cold? flu?)
Answer: my daughter she is 3 years old, but her cold is now under control.

DO I need Vitamin D supplement, Do I need to diagnose for Anemia?

Hi back,

Sorry for the late reply due to XXXXXXX clinical overload during these days.

Thank you for providing the requested information.

Overall, there is nothing worrisome with your case (taking into account your medical history and lab reports).

It's true that I found changes suggesting anemia, hoping there is no source for bleeding in your case (like upper GI or anal bleeding especially if you admit to suffer of hemorrhoids?). Otherwise, diet is extremely important to dominate anemia (especially if you are vegetarian and no meat consumer).

Now, anemia is to treat at very first place. Iron supplements, containing iron, folic acid, vit. B12, vit.C, zinc recommended to start for at least 3 months. After 3 months, should repeat blood testing to evaluate the effects of iron supplementation therapy.

Next, vit. D is a bit low, can also start a course of vit. C and calcium supplement for 1 month only.

When you'll repeat the anemia testing, should measure vit.D and calcium levels too.

Don't forget that diet plays a crucial role in managing your deficiencies if no source of bleeding.

Try to avoid intake of caffeinated drinks at afternoon.
