Brief Answer:
stool impacted

Detailed Answer:
it seems you have what we call a "fecal impaction". you may have a large amount of stool right there in the rectum ready to come out, however sometimes it gets so hard that it won't come out, and the muscles from the rectum may be weak as well.
Try enemas (either fleet enema, mineral oil enemas or just warm tap water enemas), this should help to soften the stool, and hopefully make it come out. however if you are unsuccessful, you may need a rectal exam. Sometimes by just inserting a well lubricated finger (with a glove!) into the rectum, the stool can be broken down and removed. A physician can help you do this, usually either in the emergency or a gastroenterologist.
If stool doesn't come out soon enough, it can produce ulcers in the rectum from the pressure, and it can therefore bleed as well.
hope that helps. good luck.
