Suggest Treatment For Fever And Throat Infection In A Child
Question: Hello doctor,
My son had some fever around 3 times in the past 4-5 months.
The doctors have him antibiotics for throat infection and he became alright.
Recently, we noticed that his sclera has a mild yellowish tinge. Our doctor checked him in light and told nothing is explicitly visible. For our satisfaction, the doctor ordered blood test for CBC, liver function test and CRP test.
The CBC , liver function test and CRP all were normal. But, his bilirubin levels are a bit elevated. (Direct .6, Indirect 1.3 and total 1.9, where the lab normal range was 1.3).
He is fine now and with no other symptoms and doctor has suggested to just ignore the reports and he doesn't need any treatment.
Is the elevated bilirubin levels a cause of worry?

My son's age: 4 years, 9 months.
Recent history - lot of travel outside and he has joined school 6 months back.

I have gone carefully through your query and understand your concerns. In absence of derangement of other parameters in LFT I would not get concerned about the mild rise in bilirubin levels. However, I would prefer to keep him off medicines if possible, since the metabolism of those medicines form an added load on the liver. Otherwise no treatment is needed.

1. His eyes still have a yellow tinge and looks mildly discolored. We are staying abroad and came for vacation to XXXXXXX now. Had taken him for annual eye checkup, but even the eye doctor has checked him and told that nothing is abnormal and asked us to give him colored foods , since it may be due to vitamin deficiency. So, currently giving him Zincovit. Hope this is fine.
2. Also, our paediatrician asked if anyone in our family has sickle cell anemia or thalasemia to ascertain the reason for high bilirubin level. But, no one in our family has such blood disorders. Any reason why the bilirubin content is abnormal ?
3. Also, we used to do an annual deworming for our children. Shall we do it for my son, this time too ?
4. My son says that sometimes, he has a reflux kind od sensation where he feels that food comes to his throat and goes down. Do we need to give any medication/tonic for better digestion ? Otherwise, he has a normal appetite and has regular bowel movements.

2. It is good that such hereditary diseases are unlikely (in absence of family history). However, the rise in bilirubin levels is not quite 'normal'. A proper explanation needs to be derived and your doctor is trying to do the same.
3. Do not do the deworming at present. Let the present situation subside and then you can go for it.
4. No medicines needed. However, he needs to have plenty of green leafy vegetables with diet. He also needs to drink plenty of water.
