Brief Answer:
report inconsistent with the given history..

Detailed Answer:
Hello XXXXXXX Welcome back to HCM,
I've gone through your query and understand your concern,

I've reviewed the attached report which is 7 days old, it's inconsistent with the given history here..

The given report is simply indicating of some infection (borderline elevated TLC count along with raised ESR).. WIDAL is not really sensitive test to detect Typhoid/paratyphoid infection (titer of 1:80 is not clinically significant) thus it has to be correlated with clinical presentation along with the detailed history and several other investigations based on the findings..

Several details needed specifically, in order to provide a specific guidance in this regard, please go through them carefully and answer them so that some specific conclusion can be sought, i.e.-

1. When you say, you were having fever, do you mean, it was 7 days before or even before then ? try to calculate from the day when it's started and let me know in details..

2. Also, was there any chills/rigor along with the fever ?

3. Any joint pain/ rash/ headache/ body ache/ cough/cold/ congestion/ runny nose/ pain abdomen/ loose motion ?

4. What all has been done in these past 7 days, any treatment/ further investigations ? let me know in details..

5. Was the giddiness started after fever ? how long has it been ? any nausea/vomiting along with ? anything which relieves/worsens the giddiness ?

6. Any pain in any of the ear ? any ringing sounds in side the ear ? any heaviness in side the ear / blocked nose ?

7. When you say your having tonsillitis, do you mean the difficulty swallowing or Was the oral cavity examined by the doctor yet ?

8. Any hoarseness of voice ?

9. What is the present course of treatment running, let me know in details ? and how are you at present ?

10. Any other problems apart from this ?

Let me know all these details so that I can guide you more specifically..

Meanwhile start gurgling with XXXXXXX warm water and a pinch of salt three times a day..avoid cold drinks, sour/spicy foods and dairy products for the time being..

Awaiting..

Take Care
Kind Regards

Brief Answer:
follow-up consultation..

Detailed Answer:
Hello XXXXXXX Welcome back,
Thanks for following up with these details,

I've gone through the attachments as well as that prescription, considering the fact that it's going on for so long, I strongly feel you should get evaluated by a 'proper' specialist without any further delay..

You should get a scheduled appointment with either an ENT-specialist or a Pulmonologist for a detailed clinical examination of your throat under direct vision..there will be a need to undergo specific tests i.e. Chest X-ray, sputum for gram stain and AFB, Throat swab culture sensitivity etc. , management will be mostly limiting to proper antibiotics as per the findings (please don't take anymore on-off painkillers and anti-allergics, as without proper diagnosis these are just suppressing the symptoms thus making it prolonged so long, get yourself properly examined by the proper specialist and relevant investigations in this regard)..

Meanwhile, just continue with the XXXXXXX warm salt water gargling as already advised..

I'm not going to advice anything apart from this at these moment as I strongly suggest you get examined thoroughly to diagnose the exact cause behind this..

Do let me know later on how you stay, after you get yourself checked, I'll be glad to assist you further..

Next time on wards, I'd also suggest you to open this in your desk-top/laptop so that no important points get missed..

Take Care
Kind Regards

(1) Thanks again. I was busy for these 3-4 days, couldn't visit the specialist. My throat pain is not there now. Should I still visit an ENT specialist or if the problem persists again I have to visit the specialist?

(2) Also I was worried as my mind thinks negative. I have also heard that cancer symptoms appear only at the last stage!! because my far relative passed away a couple of years back due to throat cancer. He wasn't having any bad habits & he also was suffering throat pain (Unable to swallow food /water. He used to have pain while swallowing). But when he consulted the doctor, the doctor suggested him to visit cancer hospital. & he passed away within 1 year after that.

So I was worried if something is seriously wrong with my throat pain. Pls comment on both my (1) & (2) questions.

Brief Answer:
follow-up consultation..

Detailed Answer:
Hello XXXXXXX Welcome back,

First of all, you should note that these are non-specific symptoms, means they can be observed in a number of different ailments..

1. As I've already mentioned, since this is recurring again and again, You should get a scheduled appointment with either an ENT-specialist or a Pulmonologist for a detailed clinical examination of your throat under direct vision and a few relevant investigations as per the findings (regardless of the fact how you are at present momentarily)..and describe these prolonged history to him/her from the beginning so that it can be correlated clinically..

2. There's no need to think negative, stop believing in 'hear-say' things, and simply go ahead and do the needful (i.e. get yourself properly examined by the relevant specialist as already explained), not only it'll allay all these fears and anxiety but also will help in establishing the proper diagnosis which'll ensure your future well being.. your too young to think about all these..

Throat pain is among those non-specific symptoms, which can be seen in numerous health conditions (cancer is quite unlikely, so don't stress yourself)..

Wish you good health..

Take Care
Kind Regards
