Suggest Treatment For Fever, Cough And Nasal Congestion

Question: Hello Doctor,
My 2 year old son has running/stuffed nose and Cough with mucus from the past 5 days. We were giving him Brozeet-s and Coriminic as per doctor's prescription since yesterday morning. There is no fever. But there is definitely Mucus. And the doctors also prescribed Amoxicillin Calvunate which we were not giving as we are a little hesitant to give anti-biotic which may possibly be a Viran infection. But wanted to get your suggestion if we should start using the Antibiotic. As the cough and Cold do not seem to improve.

Thanks for asking on this platform

I can understand your concern.The most common cause of cough/cold with fever in this age is some viral infection and allergy/hyperesponsiveness of airways in the absence of fever.
In viral infection fever usually shows a declining trend within 3-4 days and child general activity remains normal.
Antibiotic is needed if investigations are suggestive of bacterial infection,there is no improvement in fever or if general condition deteriorates.
As you have started Brozeet-s(a bronchodilator) and Coriminic(antiallergic/decongestant) since yesterday only and there is no history of fever you can observe the baby and wait for antibiotic.
It may take 4-5 days even after giving all these cough relieving medication to see any improvement.Observe for any breathing problem or poor feeding and if you notice any consult your pediatrician.

Thanks Doctor.

We have been using the meds since Tuesday night and there is no improvement. And the cough worsened. There is no fever though.
Can you suggest if we can use Kofarest-Pd in place of Brozeet-s, If so can you advise the dosage. We used Kofarest-PD in the past and that worked for him. But not sure about the dosage. He weighs 11 Kgs and is 22 Month Old.
And also please let me know if we can wait for a day or should take him another pediatrician.

The contents of both are almost same

Detailed Answer:
Brozeet s contains bromhexin(a mucolytic)and terbutaline (a bronchodilator) while kofarest pd contains terbutaline with ambroxol,guaphenesine(mucolytic) and menthol (soothening agent).
The contents are almost same in both the medicines and each 5 ml of both the medicine contains almost same amount of terbutaline (1.25 mg and 1.5 mg resoectively).
If you have past experience of better relief with kofarest PD you can give it.The dose will be 2.5 three times a day.

Thank you Doctor. I appreciate your detailed answer.
The other question I asked in my previous thread was that the cough worsened and he is not able to sleep /eat and is miserable. Is there something that i should do. If you can help me understand the signs to look for to determine if i should go to a different Pediatrician.

And Levocet was another Med prescribed today by the doctor. Please help me understand if it helps cough.

I do understand your concern.Sometimes cough in children causes very much distress in the parents.
It's not cough which is alarming but observe if it is associated with any respiratory distress.
Regarding feed I would like to tell you that poor feeding is a very common complaint during any illness.You should observe for the urine output as grossly decreased urine output suggests poor feeding and may need to be supplemented with IV fluid.
Levocet contains levocetrizine an antiallergic and your baby is getting coriminic which also contains chlorpheniramine an antiallergic.So I won't recommend for levocetrizine as it would not add any benefit to the baby's cough.

You can observe the baby for any worsening in general condition or can seek a second opinion.
