Fevers in children of this age is usually caused due to certain viral infections. But presence of chills and rigors warrant the requirement of looking for the cause and ruling out of urinary tract infection or malaria if that you live in an endemic zone. In order to determine the cause, I would suggest you to get a routine blood count (including platelet count) as well as urine for routine microscopy and culture to be done.Depending upon the cause, it can be determined whether an antibiotic is essential.

I would not worry about the chills.

The fever needs to be controlled by the use of crocin or paracetamol and tepid sponging. Irrespective of the use of antibiotics, such infections may last for 5-10 days. It might have an alllergic component and antiallergic like cetiriazine can be given for a few days. It also takes care of runny nose. Giving steam inhalation or use of humidifiers should be helpful by helping in diluting the cough.
