Detailed Answer:
Good evening.
As I read your symptoms I am struck by the symptoms of a sensory nature suggestive of some form of demyelination which could be of either a CENTRAL (Brain/spinal cord) or peripheral nature (beyond the spinal cord at the level of the end organ supplied by the nerve such as the tongue, oral mucosa, teeth, skin.
If I were your neurologist I would get an updated MRI scan with GADOLINIUM contrast to try and identify any type of subcortical disease whre myelin is being lost for some reason. The idea of some type of autoimmune disease process such as Lupus, Vasculitis, or other forms of collagen vascular disease, or auto-immune (Sjogren's, etc.) is less likely if symptoms are strictly in the head and upper torso....but I would still do a fairly complete metabolic workup to include thyroid function panel (TSH, FT4, FT3, TT4, TT3), Vit. B12, Vit. D, Folate, ESR, CRP, and other immunological and auto immune markers.
If beta blockers didn't help the tremor then, then this may be some type of basal ganglionic degeneration which could cause a motor tremor solo without affecting much of the sensory systems.
I'm not sure I would put FIBROMYALGIA very high up on the list of likely primary diseases since it was diagnosed 10 years ago before any of this other stuff started to happen and in fact, is more likely to be related to these other numerous system that are affected. What was interpreted as a systemic pain syndrome may have been the prelude to the sensory demyelinating/neuropathic symptoms (if in fact, demyelination is going on).
