Question: myself XXXXXXX XXXXXXX 43 years male,suffering from hypertension and neurofibroma.I am taking Sartel-40,Cinod -5 and Rosuvas-5 daily for hypertension under the supervision of an cardiologist.I am suffering from constipation since my childhood.but for past 3 years,it has almost been regularised.I started to take plenty of water,fibrous food and maintained a fixed time for bowls clearance.But i noticed some problems:
1.stool is sometimes very hard and othertime it is very soft. indigestion occurs very frequently.
2.sometimes during passing stool a burning sensation occurs at the mouth of the rectum which goes away on washing with water.
3.Sometimes after passing hard stool,a burning feelings on washing occurs at the rectum .For this problem
i visited my general physician in the month of may,this year,who checked my rectum thoroughly and by inserting his finger.after checking he stated no problem is there.After few days,when same problem occurs,i consulted one general surgeon and after checking he stated that a mild anal fissure is there.he adviced me to take hot water vapour adding betadine lotion in it followed by application of sucral ano cream at the affected area.
it gives me a relief.It happened in the month of september,2014.Yesterday,after passing hard stool,a feeling surrounded me all the day.Something as if a sharpen end trying to come out from rectum which is very similar feeling whem worm develops.sometimes an eatching like feelings was also occuring on the mouth of the rectum.
4.today,my stool was quite soft.still,when it was coming out,a feeling occurs inside the rectum as if one pin is inserting on the wall of rectum. It occurs somewhere inside the rectum.It could not be called very painful but it was felt.I felt it before when hard stool moving downward but this is first time i felt it despite soft stool.
this feeling occurs sometimes but does not occur frequently.
I could not find any worm in my stool by nacked eye.moreover,today i did not felt any such feelings feelings what was there yesterday on my rectum.please explain me the reason for those aforestated symptoms and their possible solutions.

the symptoms stated in point 4 is just like the stool has a pointed edge which has stricked at the wall of rectum during it's downward movement.it is felt when stool is hard.but it occurs today despite the stool being soft-why?

i am suffering from tremendous tension and panic for different reason,mainly for health reason.
