Suggest Treatment For Frequent Bouts Of UTI

Question: My father aged 74 years was diagnosed with a moderately enlarged prostate last year.
A local surgeon recommended surgical treatment and as a temporary measure prescribed the drugs Alfatam or Silodal .
Because these drugs had severe adverse reaction he stopped taking these after one single dose and decided to live with the ailment instead of living with the effects of drugs and or after effects of surgery.
As on date he does not have any complication or extreme discomfort (Such as frequent or painful urination ) except an occasional bout of UTI for which he takes a short course of antibiotics such as Zenflox or Mortifur etc. He has had bouts of UTI for the past fifty years ever since he had a minor accident while riding a cycle.
We would however like to go for a detailed medical examination to check for possible complications such as impending prostate cancer or any other serious ailment in course of time.
I am attaching the first clinical report of his condition for your scrutiny .
Can you please suggest the various test that need to be carried out before deciding on a suitable line of suitable treatment.

Hello thanks for query.

Based on the facts and reports of your father that you have posted your father has obstructive urinary symptoms due to bladder outlet obstruction due to Enlarged Prostate .

In view of presence of Diverticulum in bladder and predominance of median lobe he is unlikely to respond to medical line of treatment .

Please consult qualified Urologist and get following basic tests done
1) Urine routine and urine culture
2) Ultrasound scanning of abdomen and pelvis (Fresh one)
3) Trans Rectal Ultrasound Scan.
4) Serum PSA.

I would suggest you to get endoscopic surgery of TURP done to resect enlarged prostate

This is gold standard method to resect the enlarged prostate practiced world wide.It has low morbidity and offers permanent cure and improves quality of life at this age .

Thank you for your reply. I have one more question to ask.
As per MAYO clinic and several other web sites TURP involves certain risks such as:
Heavy bleeding
Temporary difficulty urinating.
Difficulty holding urine plus several others
Erectile dysfunction
Need for re-treatment This risk is disconcerting

He does not suffer from any of these at the moment except as mentioned below only UTI if the body gets dehydrated. Otherwise no problem.

He feels a surgery such as traditional TURP could make it difficult for him at the moment . A suitable antibiotic to tackle UTI when it turns up may be a reasonable compromise. If you can suggest some thing it might be helpful

He is looking at the possibility of going for Holmium laser treatment(HoLEP) , if the problem escalates

Can you please something about this operation and the after effects of such a surgery. I believe there are several hospitals in XXXXXXX who specialise in this kind of operation.

It is your misunderstanding about TURP.

Thanks for follow up.
With advanced equipment and optic system there are no cases reported about heavy bleeding in recent times Rest of the complication that you have mentioned are very rare and even they occurthey get resolved within 3-4 weeks .

As regards Holmium laser the equipment and technique are same as TURP only difference is in TURP conventional electric energy is used to resect prostate where as in Holmium laser technique laser energy is used to burn prostate tissue .Rest of the complications that you have mentioned are similar to TURP.

In case he develops UTI please give him antibiotics as per culture report.
