Suggest Treatment For Frequent Idiopathic RVOT PVCS And Abnormal Heart Rhythm

Question: Hello Doctor , I am a 22 year old guy with a history of frequent idiopethic RVOT PVCS . I have had an catheter ablation 7 - 8 months ago and my PVCs from 15 000 a day gone down to sometimes none sometimes 20 - 25 a day . So my doctor said ablaton seems to have a very good results but recently twice in the past week have felt something that does not feel like a PVC or a PAC , it last for 3 - 4 seconds and it is a very scary sensation , if I had to use one word to describe I would say , , Quivering '' ?
Feels like my heart shivers , shakes like crazy and goes back to beating again , like i have said before I am used to PVCs and PACs and this doesnt feel like them at all ? All the tests on my heart , echos , stress test , MRI have been normal exact slightly enlagre Left Ventricle 9ml above normal size , wall thickness and mass within the normal rage just the size was I believe a bit bigger than normal during my MRI scan . Anyways I just want to ask you about this quivering feeling , do you know what it could be ? Multiple PVCs PACs together ? afib ? VT ? Or fibrilattion ? I am so scared i was thinking about going to the ER ? Thanks

I read your query and understood your concerns.

You are experiencing a quivering sensation this last week. It most probably is multiple PVC or a stress and anxiety related issue. However anxiety is the last diagnosis after all cardiac examinations (especially Holter monitoring) will result normal again.

I would like to know when was done the last Holter monitoring. If it was done up to sixth months ago it should be done again in order to find out exactly what's going on with your heart rhythm.

If this quivering sensation is going to be more frequent or intense you should go to ER to exclude any problem, otherwise you should plan a cardiologist consultation and a Holter monitoring.

The last 48 hour holter monitoring was done 1 months ago , it showed A few PACs and PVCs , i also have done 30 event monitor three months ago with the same results we got isolated PVCs and PACs sometimes three together , so i guess that's called SVT ? During my ablation my heart went into Afib ? And later my doctor said it was not going to happen outside of the EP lab that catheters caused it . Anyway so far after the ablation my PVCs have been gone but now this quivering a very different sensation scares me to death ! It did not make me dizzy or anything and it lasted 3 - 4 seconds , could that be Vent Fib ?

Hi back,

Based on the new data you provided your symptoms may be related to SVT and you shouldn't worry about SVT. Appart from the quivering sensation SVT wouldn't cause any harm to you.

As long as no other arrhythmias were detected on your holter you should stay relaxed. Moreover you shouldn't worry about ventricular fibrillation. It's not a self limited arrhythmia and it requires cardiac defibrillation to restore the sinus rhythm.
