Brief Answer:
You need optimization of medicine for diabetes.

Detailed Answer:
Hi Mr. XXXXXXX

Thanks for your question.

After going through your question, I can understand your concern.

There may be following possibility for your symptoms of frequent urination-
1) Uncontrolled Blood sugar levels:
This is most likely cause in you.
Your blood sugar control is not up to the mark.
Blood sugar target for you Fasting below 130mg/dl and Postprandia less than 160mg/dl.
Though your mentioned figures look closer to target, HbA1c level will give better idea of control of diabetes.

2) Urinary infection.
This looks unlikely in you.
Though rare in male but risk increases with presence of diabetes and further increases with uncontrolled blood sugar level. Usually present with fever.

I will advise you to do-
- HbA1c level
- Urine examination.

For better control of blood sugar levels follow-
1) Strict diet control.

2) Regular exercise at least 150 min per week.

3) There is need for addition of antidiabetic medicines along with current Tab Gluconorm SR 1gram (which contain metformin) twice a day.

I will recommend you Tablet containing Combination of Metformin 1 gram and Sitagliptin 50 mg before breakfast and Continue Tab Gluconorm SR 1 gram after dinner (Considering normal heart, liver and kidney function).

Repeat Fasting & Postprandial blood sugar level after 10 days and if required increase the dose of Tablet containing Combination of Metformin 1 gram and Sitagliptin 50 mg twice a day.
These changes should be discussed with your diabetologist.

Depending on urine examination report need for antibiotic can be decided.

Hope I have answered your question.
I will be happy to answer your follow up questions if any.

Hi Mr. XXXXXXX

Thanks for your follow up question.

Tablet containing Combination of Metformin 1 gram and Sitagliptin 50 mg is Tab ZITAMET (50 + 1000) you can take it once a day before breakfast.
Continue Tab Gluconorm SR 1gm once a day after dinner.

Sitagliptin do not affect liver or kidney function.

Brief Answer:
Vitamin D deficiency.

Detailed Answer:
Hi Mr. XXXXXXX

Thanks for your follow up question.

I want to appreciate you for your effort towards diabetes control.

I feel really happy to see your HbA1c level of 6.8 (which indicate really good diabetes control).
Considering your current blood sugar control and medications for diabetes, you can achieve even lower HbA1c value.
Continue your current efforts for the same.

Though your cholesterol level is under control, I will recommend you to use Tab Atorvastatin 5mg after dinner (Statin).
Usually in all diabetic patient after age of 35 years it is recommended to take statin even with normal Lipid level (only dose varies with LDL cholesterol).

Vitamin D level of 4.8 indicate vitamin D deficiency.
I will recommend you to take XXXXXXX or tablet containing containing Vitamin D3 60000IU (Like D rise or Calcirol sachet) once a week for 8 weeks there after once a month for next 8 to 10 months.

Hope I have answered your question.
