Suggest Treatment For Fungal Ear Infection

Question: I am in Romania. I have no tympanum in my left ear.
I have a fungal ear infection and will be traveling when culture results come in.
I am taking two topicals: Nidoflor and Betabioptal, and Clindamycin-MIP 300 mg/8 hrs.
I'm going to be away from home for a week and this is quite invasive and painful.
I tried Exoderil first and it really burned and I stopped.

Voriconazole is available OTC, perhaps in oral and topical form.
I'm thinking it's a good way to stave off the fungus while I'm on the road.
Thoughts?

Stop Betaboptal, other medications are fine. Ear cleaning helps.

Hi,

Thank you for your query.

1. If you have an ear drum perforation on the left side, stop Betabioptal (Chloramphenicol) which is ototoxic.

2. Most Antifungals are not ototoxic, hence you may continue the rest of the current treatment, which-ever is readily available including Voriconazole. Avoid Exoderil.

3. In Otomycosis (fungal infection of the ear), regular cleaning of the ear by an ENT Specialist will help it heal faster. Get the ear cleaned when you get beck next week. Otomycosis can be treated aggressively if the ear drum is intact. An ear drum perforation leads to a wet external ear, which helps the fungus grow.

4. In the long term, closure of the ear drum perforation will be required.

Follow up: Dr. Sumit Bhatti (7 hours later)
So I'm only using Nidoflor topical and Clindamycin-MIP 300 mg 3/day for 5 days.
When I stop the Clindamicyn I will only be taking the Nidoflor topical in the external ear.

I'll get it cleaned regularly on return.
Anything else I could be doing?
I want to schedule surgery to restore my ear drum in early January.
Does everything have to be fully healed by then?

Yes, that is fine. Wait minimum 3 weeks before surgery.

Hi,

Thank you for writing back.

1. Yes, Topical Nidoflor continued after the Clindamycin course is over is fine.

2. Do get a regular ear suction clearance done. Removal of fungal debris from the ear promotes healing.

3. Wait at least 3 weeks after the ear is completely healed before going in for ear surgery. The ear should be dry. This will help get better results.
