Brief Answer:
need antifungal medications

Detailed Answer:
Hello and thank you for your question.
I have read your question and looked at the photo of your nails that you included. It does look like a nail fungus but you would need to see a doctor and have a bit of the nail removed and sent for a culture before you could be treated for toenail fungus.
There are other causes of thickened toenails.
The reason for this is that toenail fungus does not respond to topical medications and you would need to be on oral tablets if this is indeed a fungal nail infection.
Oral antifungal medications can be toxic to the liver so your doctor would need to make sure your liver tests are normal before starting the medication and would need to monitor liver blood tests during the course of treatment.
Funguses are yeasts that are in the environment pretty much everywhere and we do not know why some people get infections with them and others do not.
I wish you the best,
Dr. Robinson
