Suggest Treatment For Glans Penis Lesions

Question: Dear Doctor,
I have been suffering from glans penis lesions (red spots ) since the last six months.
The problem started 1 week after I had protected sex with an unknown person. I had used 2 condoms but still I think my condition is due to having sex with that unknown person. Slowly the glans penis was covered by many red lesions/red spots similar to pimples.
After 3 months of this sexual contact, I have done all the STD tests and they have come negative.
I have been consulting a well known skin specialist who is confident that it is a fungal infection. He says there is no need of any further tests and has been giving me anti fungal creams but still the condition has not gone completely and there are still a few (two -three) red spots on my glans penis. Now every day twice I am applying coconut oil on my glans penis to manage the condition as it has anti-fungal properties. After taking 2 Flucos 200 mg tablets per week for 2 week, the conditions has improved significantly but still there are 3-4 red lesions.
Kindly advice as I am planning to get married next year and do not wish to pass on the infection to my wife or also affect my child's health when he/she is born.
Problem summary:
3-4 Red lesions/spots on glans penis.
No pain.
No secretion/discharge.
Sometimes glans penis is very dry and wrinkles.
Sometimes the glans becomes very shiny.
I am uncircumcised.
Kindly let me know if more information is required. It has been 7 months now since this problem. Kindly advice how to treat this condition.

I am Dr. Kakkar. I have gone through your history, which seems largely uneventful in terms of an STD and I have also seen the photographs.

I completely agree with your doctor that you don't need any further testing for STI's as you have already tested negative for STI's at 3 months after contact which can be taken as conclusive (I hope you have tested for common STI's like HIV, Herpes, VDRL/RPR?)

Coming back to the red spots on glans, this does'nt seem to me an STD. This is most likely a mild fungal infection Or a mild irritant dermatitis to soaps, antiseptics etc that you may probably be using to clean this area.

I would like to clarify, how do you clean this area? and whether you use any soap or antiseptics like dettol, savlon etc over here?

Thanks Doctor for the prompt reply.

I am not using any kind of soap or antiseptic or body wash to clean the area since the last 3 months as the same was advised to me by my dermatologist.

I only retract the foreskin while bathing and wash the area with warm water twice daily. After bathing, I dry the area under ceiling fan by retracting the foreskin and then apply 4-5 drops of coconut hair oil (Parachute).

Kindly advice how to completely treat this infection so that it does not recur again as I am afraid that if I stop using coconut oil, I might get the red spots again covering my glans completely.

I need help to completely treat this condition as I am afraid that even if small infection (fungal as you say) is there, it will again grow under favorable conditions and spread to other parts. Kindly advice so that the infection is completely killed so that it does not recur and I can lead normal life again without any mental agony.

Hi.

Thank you for the clarification.

I would suggest you to use an OTC topical antifungal e.g clotrimazole 1% cream, twice daily for a week.

I would rather ask you not to use coconut oil, as it might rather encourage candidal growth because of the occlusive moist conditions it creates, that might encourage candidal growth. The single most important factor for candidal growth is occlusive moist conditions that exist in body folds and coconut oil would further encourage that. It is a common practice, in XXXXXXX to apply coconut oil for a lot of skin condition, but it is ill found.

Just use a topical antifungal and nothing else. It should be fine within a week.

No, it won't spread to other areas
