Suggest Treatment For Growth Difficulty In A Child

Question: My son is 14 years old.He has growth problem.His height is 141 cm.I have got his GH TEST and IGF 1 TEST done. His IGF 1 test value is 167 ng/ml.His GH test value is .52 ng/ml at 0,.7ng/ml at 30 minutes,3.18ng/ml at 60 min.,3.36ng/ml at 90 min.Please advise me.

Hi....I understand your concern. Before commenting on the test reports I need more information. Please answer the question below -

1. What was his birth weight and length (if documents are available)?

2. What are his current weight? (Height you have already mentioned as 141cm)

3. What are the heights of both parents (mention individually for mother and father)?

4. What is his exact date of birth?

5. At what age did either one of his parents start growing up in height (for example I am asking is whether if either or both the parents had a growth spurt after 14-16 years of age)?

6. Are the serial growth hormone values you have mentioned - are the results of grwoth hormone stimulation test?

7. The growth hormone stimulation test values can be different in different labs. I would like to know the normal values for this particular age and time in the lab in which you have done the test?

This test is most often done to find out whether GH deficiency is causing slowed growth.

Normal Results usually are -

Normal peak value -- at least 10 ng/mL
Indeterminate -- 5 - 10 ng/mL
Subnormal -- 5 ng/mL

(A normal value rules out hGH deficiency; in some laboratories, the normal level is 7 ng/mL.) But your son's values are below the normal range.

Normal value ranges may vary slightly among different laboratories. Some labs use different measurements or test different samples. This is why I ma requesting you to get back with the normal reference ranges of this particular lab.

Along with this information - please get back to me with answers to all the above question please.

He needs evaluation and GH deficiency is a definite possibility

Detailed Answer:
Hi...your son needs evaluation for growth hormone deficiency. Though his mother has attained her growth spurt at a later age, we cannot sit on this alone and wait. I have calculated his mid parental height which is coming to 160.5 cm. When extrapolated in comparison with this- his current height is not falling into this extrapolation.

Moreover though his growth hormone stimulation test is normal - his basic GH level and IGF levels are low.

So this is the reason why I feel that he is GH deficient and needs further evaluation.

He will require an evaluation and follow up with a paediatric endocrinologist with experience in growth hormone management.

If it would confirmed that he has growth harmone deficency and remain untreated then what would be his expected height.
If deficency confirmed then which treatment we should follow?

Untreated height cannot be expected

Detailed Answer:
Hi....I'd there is no growth hormone deficiency then the expected height would be 170.5cm.

But as there is growth hormone deficiency- its difficult to estimate the expected height now.

If deficiency is confirmed then the treatment that should be followed is growth hormone replacement therapy which is very costly and should be used only under the medical supervision of an endocrinologist.
