Question: Hi my daughter is 15 yrs old.she had passed hard stool since a week with blood.pls advice me.

Hi XXXX! Good evening. I am Dr Shareef, a general surgeon answering your query.

With the history, it seems that your daughter has got a fissure in ano, which is a wound on the mucosa and skin of the anal canal due to passage of hard stool.

If I were your doctor, after a physical general and local (per rectal)examination to confirm the diagnosis, I would advise her with a stool softener, a broad spectrum antibiotic active against both aerobic and anaerobic organisms, an anti inflammatory drug along with a proton pump inhibitor, an ointment containing a local anaesthetic, a muscle relaxant and possibly a mild steroid to reduce inflammtion on the wound, and advise her for sitz bath (sitting in warm water tub) three to four times a day. This should relieve her symptomatically and would possibly give her a cure provided she regulates her diet to avoid any kind of constipation. If these measurs do not help her, then she might need a surgery after being examned by a surgeon.

On the dietary front, she should go more for high roughage diet like green vegetables, salads, fresh fruits, and plenty of liquids, with avoidance of high protein diet and fast food which would precipitate a constipation.
