Hi male patient at a age of 63 presenting with complaint of presyncope,fatigue malaise,lassitude,cold feet and ankle can be most likely -
1)Congestive cardiac failure
2)Presyncope
3)stroke
Congestive cardiac failure is a condition in which feart fails to pump blood required by body to perform normally,As different organs are not recieving blood properly ,symptoms like fatigue,malaise,lassitude,letharg starts appearing,cold feet and ankles can be also because of improper blood supply to the periphery;as maintaining temperature of the body is also a work of blood.If blood supply to a part is improper it will be cold and clammy.
Most common cause of congestive cardiac failure in adults is coronary artery disease.
I would suggest you to go for a base line test of cardiology called 2d echo,to rule out possibility of congestive cardiac failure due to ischemia of myocardium.
Secondly it could be a condition called presyncope-Presyncope is a state of lightheadedness,dizziness, muscular weakness, blurred vision, and feeling faint. Presyncope is most often cardiovascular in etiology. .To rule out possibility of presyncope ;Carotid artery doppler should be done to find out if blood supply to brain is normal,In old age due to plaque formation carotid artery is occluded hence blood supply to brain is hampered leading to the above mentioned symptoms.
Stroke-All the symptoms mentioned by you can be because of stroke also,association of symptoms with headache makes it essential to rule out stroke as a cause of your symptoms,However stroke i usually associated with mild paralysisor weekning of limbs;I would advice you to go for a MRI brain stroke protocol.
Its highly unlikely that cirrhosis is associated with your symptoms
