Question: Dear Doctor,

I am 31yrs old male and struggling with heart burn for past few days. I am taking Pan D tablets.I have chest burning along with radiating pain from left jaw, left side of the neck, left shoulder down towards left hand, feel like something stuck in throat, dry cough. Also whenever i have heart burn or digestive related issues, i feel like the left side of the body is heavy. Is these symptoms are related with heart burn or something else?
How to treat them? I have these heart burn issues on and off for more than 5 years. But it is getting worse now a days.

Brief Answer:
cardiac workup, if negative UGI endoscopy

Detailed Answer:
Hello,
I have gone through the description you have provided.
GERD ( gastro esophageal reflux disease) could be responsible for all of your symptoms.
But I would suggest cardiac work up, as I would like to rule out heart disease in view of the left jaw and left hand pain. This workup would include a 2 d echo and an ECG.

In case the above tests are negative then the next step would be to get a UGI ( Upper Gastrointestinal) endoscopy to look for erosive GERD.

I would suggest to increase the dose of pantoprazole to twice a day.

I would advise to avoid Alcohol, meat, mint and chocolate flavoured foods, citrus fruits, deep fried foods.

I would suggest not to lie down for 3 hours after meals.

Hope this helps
