Detailed Answer:
Hi, thanks for posting the concern in the HCM.
As I can understand from your query, you are a 30 year old female with history of wheezing (for which you have been taking some inhalational medication since 2007) and has present complaints of restlessness and palpitation for last 3 days. There is no history of any chest pain.
From the history above, it seems to me that you are having some form of arrhythmia. This is alteration of the regular rhythm of the heart that is generated from SA node of the heart. This may be due to numerous reasons including cardiac and non-cardiac causes. Now, what you need to do is two things-
1. Consult your local cardiologist for clinical examination and further evaluation
2. Do all the investigations to find out the underlying cause
Now, I will provide you the list of investigations that I usually request my patients to do in similar situation-
1. Blood for Complete blood count, urea, creatinine, electrolytes, serum calcium, HbA1c, fasting TSH
2. ECG-12 leads
3. 24 hr Holter monitoring
4. Echocardiography
In case of non-cardiac reasons, correction of the underlying problem, e.g. electrolyte imbalance, thyroid disorder, anemia etc. will cure the problem.
In case of cardiac conduction defects, antiarrhythmic medications and in certain cases pace-maker implantation may be needed.
As you have wheezing problem, your lung status and oxygen saturation should also be evaluated.
Therefore, an arterial blood gas analysis may be needed alongwith a chest X-ray-PA view, if found necessary after clinical examination by your local doctor.
You can get back to me after having the investigations done and consulting your local doctor for any further help.
