Suggest Treatment For Hemorrhoids

Question: I had an itchy anus a few weeks ago, and didn't get it checked out. I started to get small bumps, and these have gotten larger and finally decided I needed to get it checked out. It is itchy, but not painful. It has bled slightly when I had diarrhea (I think unrelated) from wiping a few days ago. I have looked a lot online and am imagining it could be HPV, anul warts, but of course hoping it isn't and that it could be hemarrhoids. I will see a specialist as soon as I can and wanted to ask some questions in the meantime. I also have blood coming from my mouth each time I brush my teeth, and wondering if this could be related. This has been happening for about 2 months now. - can you tell from the picture what it could be? - if it was HPV anal Warts, how long till it could become cancer if they aren't removed? (I am sure this is a long time and unlikely but would like to calm my anxiety around it) - should i immediately get them removed or could this happen in a few weeks? (i'm in the middle of a project right now demanding all my time) Thanks

I am a Gastroenterologist and would be addressing your concerns.
Have seen the pictures attached.
As per your queries answers are-

As per your history and the attached image, your case appears to be grade 3 hemorrhoid (i.e.hemorrhoids prolapse and usually require manual reduction).

Blood in gums after brushing is due to degiciency of vitamin C so start taking Vitamin C 500 mg once daily for 1 month and get your vitamin C level checked.

Its not related to hemorrhoid.Its not HPV or warts.
As per the protocol stage 1 to 3 are managed conservatively while surgery is done on stage 4 hemorrhoid.

Suggestion for conservative management are-

Start taking Polyethylene gylcop 10-15ml once daily before going to bed.Its a laxative.Straining while defecation can make things worse.

-Secondly, Start using PreprationH a combination containing contains Calcium Dobesilate, Hydrocortisone, Lignocaine, and Zinc.

Thirdly, request your gastroenterologist to prescribe you with oral Calcium Dobesilate-Doxium therapy for 3 months. Initially twice daily for 21 days followed by once daily for 10 weeks. Trials have shown that 3-month therapy can cause shrinkage of Hemorrhoids in almost 60% cases.

Add Metamucil/Fybrogel to your regimen. It's a natural laxative containing husk. It absorbs water from the intestine and swells up making passage of stool easy and add bulk to stool.

-A probiotic should also be taken regularly with these medicines. Probiotic improves overall digestion and relieves constipation.

However, if not relieved even after 3 months of aggressive therapy proceed for Rubber bands ligation- inexpensive and are easy to deploy. It's a daycare procedure usually done under local anesthesia. The success rate varies from about 70-80%.

Have you ever had a colonoscopy done? It is recommended that all people should have a colonoscopy every 5-10 years. Even if you have had this test in the last 5 years it might be recommended to repeat it.

Initially go for medical management for 3 months as suggested above if not relieved then non invasive methods like rubber band ligation could be used.

Hi,

Thanks so much for such a detailed reply, this is great to hear. I'll be checking in with a gastroenterologist as you say and will relay this.

Just one last question, may I ask how you are sure it's not HPV and that it is definitely haemorrhoid? There's a big visible difference between the two?

I am sorry for my diagnosis.
Sometimes its very difficult to make a diagnosis without physical examination.

If HPV is confirmed then this wart would be removed using podophylline and anti HPV vaccine named Gardacil should be taken by you on 1,4,6 th month respectively.
