Suggest Treatment For High BP

Question: Hi Doctor,

I am working in IT (under pressure) environment, hence I go every quarterly to cardiologist for regular checkup, it has been 10 years regular checkup at times I had high BP (150/90 -160/90), doctor never prescribed me medication during these years I underwent 4 time stress test (last one was three years before). Everything was normal, last two week I was feeling more stress and I went to doctor for checkup, he has asked for a ECHO test and told me that you have mild LVH (1.1cm) but this is not significant and your BP is 155/90 and we need to control it. At present he has prescribed me Olemesartan Medoxomil/Amlodipine Besilate (40/5 mg). At present I am working aboard and my friends asked me to go back to my country XXXXXXX I need your suggestion. (My age is 43)

Cardio said everything is normal and the report is not yet report out. My present weight is 97 kg and I am started to work out since last week.

I understand your concern, and would like to explain that regarding your actual clinical scenario, there is no evidence for any serious ongoing cardiac conditions.

You do not represent clinical symptomatology of any coronary syndromrs, heart failure, rhythm or conductance abnormalities. So relax!

The only issue to be aware is your high blood pressure. You need to continue your prescribed therapy and to be very meticulous on monitoring your BP values.

The other challenge is to control your potential coronary risk factors (dyslipidemia, glucose intolerance, smoking, etc), and of course your over-weight condition.

You need to follow a healthy lifestyle (healthy diet and physically active profile), and a periodically scheduled medical follow up.

Thank you for your reply. In case my mild LVH becomes moderate/severe LVH, is pacemaker shall help me? What is mild, moderate and severe rating of LVH.

An isolated 1.1 cm interventricular septal thickness is within normal ranges. You don't even have to think about LVH.

Returning to your question about LVH grading, I would explain as follows:

If i/ventricular septum and LV posterior wall are 11-13 mm, mild hypertrophy is considered, 14-16 mm is moderate, and greater than 17 is severe.

Please could you present an echo report (upload), for a better review of your situation.

If you leave untreated your hypertension, then LVH may be a future consequence.

So, always keep an eye open on high BP.

Regarding pacemaker discussion, it doesn't seem to be an option for LVH.

Thank you dr. I shall get my report on Monday, meanwhile I put myself on diet, I started speed and brisk walking for 30-40 mins. I think my BP is under 135/75, at times occasionally it moves to 145/75, this is post medication which was stated since 4 days.

Along with it can I have sex with my spouse? Is it a problem?

I got my echo report whereas it shows 1.2 cm and the images shows 1.27 cm. Actually it is not 1.1 cm as previously told. I asked Dr. again today he was upset and he told me that I have made mistake of explaining this to you. you are okay just keep the BP under control, visit me every month. You pathology of heart is very good. So need to worry. you need to worry when it reaches more than 1.5 cm. I need assessment of the report as well as the images for your review.

How long shall it me to bring it back (< 1.0 cm) to optimal health condition.

I reviewed carefully your Echo report, and would like to explain that your mild interventricular septal hypertrophy has not any clinical significances at all. it doesn't imply any adverse consequences on cardiac structure and function.

So, don't worry about it, whichever be the exact value 1.1 cm, 1.2 cm, or even 1,27 cm. You have the opportunity to stop a further progression of septal hypertrophy by optimal control of your BP values.

If you continue to treat propertly your BP, potential hypertrophy with not progress, and there are also good chances to expect a hypertrophy reversal within the first year of optimal treatment.

I have heard from people that people who has LVH more than 1.50 cm also has normal life for many years. Is this true? I am worried and more than me my spouse is worried so much? At home we don't have pleasant feeling. Please suggest.

You are right. LVH is well tolerated for many years. You don't have to be worried at all. Your Echo report demonstrates a normal functioning heart. You have only a mild left ventricular hypertrophy, that may be controlled with a regular antihypertensive therapy. I would recommend nothing else, just the above mentioned advices.
So treat appropriately your high BP values, to maintain the optimal levels. Follow a healthy diet, avoid smoking contacts, and other coronary risk factors.

So far my BP is good within 120-130/70-75 but this without saly intake. I afraid if start talking half salt or less sure the BP shall elevate. I can't take alternative to sodium. What do you suggest? In tgis case i may need little more antihypertensive medicine.
