Blood pressure should not be lowered quickly at this stage. If your brother was already using anti hypertensive medication then the dose can be increased accordingly to have better control. At age 67, controversy still exists as to treat the systolic hypertension i.e the 175 or not as isolated systolic hypertension is considered by many to be physiological and aggressive lowering of the blood pressure can be counterproductive. The diastolic level 119 is high and should be treated. If there is no evidence of evolving end organ damage then a calcium channel blocker like amlodipine can be given. You mentioned being in the E.R, there are many other factors that have to be considered. If there is fluid overload associated with the condition then diuretics would also be required. In case of shortness of breath or history of asthma, beta blockers especially non selective ones should be avoided. If the rise in BP is acute and there is evidence of malignant hypertension not responding to conventional medication then labetalol nitroprusside and nitroglycerine can also be utilized. I am sure the emergency team will have these points in their mind.
